Additional Updated (Bivalent) COVID-19 Booster Dose Now Available to Select Individuals
- Individuals 65 years of age and older who have received a single dose of the updated (bivalent) vaccine may receive one additional dose at least four months following their initial bivalent dose.- Most individuals, depending on age, previously vaccinated with a monovalent COVID-19 vaccine who have not yet received a dose of a bivalent vaccine may receive a single dose of a bivalent vaccine.
- Most individuals who have already received a single dose of the bivalent vaccine are not currently eligible for another dose.
Green County Public Health is located in the Green County Government Services Building (N3152 WI-81, Monroe, WI 53566). Please use the main entrance. We are able to vaccinate individuals 6 months of age, and older.

If you are attending a clinic to receive a second dose or booster dose, please bring your CDC COVID-19 vaccination card with you.
All individuals will need to complete a COVID-19 Vaccine Administration Form prior to receiving the vaccine This form will be available at our clinics, but you can also print the form below, fill it out, and bring it with you to the vaccine clinic. Please also review the Emergency Use Authorization fact sheet (see below) of the COVID-19 vaccine you would like to receive if you plan to attend one of our clinics.
