COVID-19 Vaccine

Fall 2023/Winter 2024 COVID-19 vaccine is recommended for everyone 6 months and older.

As of 9/11, the U.S. Food & Drug Administration approved and authorized emergency use for updated COVID-19 vaccines formulated to more closely target currently circulating variants of COVID-19. The CDC recommends everyone age 6 months and older get a fall 2023 COVID-19 vaccine. The COVID-19 vaccine is now commercialized and is covered under private health insurance. Community members with private insurance can check with their healthcare provider or local pharmacist about getting an updated COVID-19 booster. Green County Public Health will be able to offer the updated COVID-19 vaccine to individuals who are uninsured or underinsured. Please see the list below for more information on who is eligible to get vaccinated at Green County Public Health:

Vaccine for Children (VFC) Program Eligibility
- Medicaid-eligible children
- Uninsured or underinsured children. Underinsured are children with health insurance, but coverage does not include vaccines. 
- Alaska Native or American Indian children

Bridge Access Program Eligibility
- Adults 18 years and older without health insurance
- Adults whose health insurance does not cover COVID-19 vaccine

All individuals will need to complete a COVID-19 Vaccine Administration Form prior to receiving the vaccine This form will be available at our clinics, but you can also print the form below, fill it out, and bring it with you to the vaccine clinic.
